In response to numerous requests from member hospitals, PHAQ commissioned Deacons to develop a generic template of By-Laws and Professional Guidelines for Medical Practitioners and Allied Health Professionals. Whilst these By-Laws make specific reference to Queensland legislation, it is considered that minimal customisation would be required for use in other States and Territories. As a service to the private hospital industry, PHAQ is now offering this template at low cost to non-PHAQ members.

What does it cost?

The By-Laws are available in word format on a CD-ROM to enable facilities to easily customise to suit their own use.

A tiered pricing structure applies as under:
  • Single Site Facility - Not part of a Group $ 385 (inc GST)
  • Part of a Group - Less than 5 facilities $ 770 (inc GST)
  • Part of a Group - 5 or more sites $1400 (inc GST)
